The Forfait Express Slimming is unusual among Bali body treatments in that it does not rest on a single machine. It stacks the Cold Lipo Laser, ultrasound cavitation, cryolipolysis and mechanical lymphatic drainage into one session, and the order is deliberate. What follows is an honest account of what each stage does to fat tissue, how good the evidence is for each one, and where the reasoning runs ahead of the published research.

Stage one: the Cold Lipo Laser, and an honest word about how it works

The session opens with low-level laser therapy, applied across the whole body rather than a single zone. In the peer-reviewed literature these devices emit red light in the region of 635 to 680 nanometres. This is not a heating laser and it does not cut; the light is delivered at an intensity that leaves the skin at normal temperature, which is why the treatment is described as cold.

The opening stage: red-light pads worked across the whole body, the skin staying at normal temperature.

The claim usually made for it is that the light briefly increases the permeability of the fat cell membrane, allowing stored triglyceride to move out into the surrounding space. That idea comes from a 2002 paper by Neira and colleagues, who irradiated fat tissue and reported transitory pores in the adipocyte membrane under electron microscopy. It is a genuinely interesting result, and it is also the most contested thing in this article. The tissue was studied in a laboratory dish rather than in a living patient; an independent group was unable to reproduce the pores; and a comprehensive review of low-level laser for fat reduction calls the mechanism controversial and concedes that the microscopy findings may be an artefact of specimen preparation. A later imaging study argued directly against the pore model.

What is better established is narrower and more prosaic. Short sham-controlled trials do find a small but measurable reduction in tape-measure circumference over treated areas, which is the basis on which the FDA has cleared this class of device for non-invasive circumference reduction. Researchers have also proposed that the effect may work through systemic changes in lipid metabolism rather than local injury to the fat cell at all. The honest summary is that the laser stage does something measurable, and that the profession has not settled on why.

Stage two: ultrasound cavitation, the best-evidenced step here

The cavitation stage: low-frequency ultrasound worked over gelled skin, one area at a time.

The Cavitation stage is where the mechanism becomes solid. Low-frequency ultrasound creates rapid pressure oscillations in the fluid around fat cells. Microscopic bubbles form and collapse, and that collapse transmits mechanical stress to the adipocyte membrane.

What this actually does to human fat has been examined directly. In a histological and ultrastructural study of treated human tissue, ultrasound opened micropores of roughly half a micrometre to one and a half micrometres in the cytoplasmic rim of the fat cell, through which triglyceride escaped. The cells were not killed. There was no apoptosis, and no damage to the capillaries, the connective-tissue framework or the overlying skin. Blood measurements confirm the mobilised fat is real: free fatty acids rise measurably after treatment, then settle.

Two things follow from that, and both matter. Cavitation empties and shrinks fat cells rather than destroying them, so language about blasting or killing fat is simply wrong. And emptying fat cells is a cosmetic change, not a metabolic one: a randomised trial in patients with fatty liver disease found that cavitation added to exercise gave no advantage over exercise alone for the outcomes that affect health.

Why the laser comes before the cavitation, and what we cannot claim about it

In the studio: one body area at a time, which is what lets each stage be done properly.

Placing the laser first is a considered choice, on the reasoning that a membrane already made more permeable should yield more readily to the mechanical stress that follows. It is a coherent idea and it is the logic behind the sequence.

It is not, however, a proven one, and it would be dishonest to present it as such. No published study has compared laser-then-cavitation against cavitation alone. A registered trial designed to test light combined with ultrasound on abdominal fat, NCT05155683, completed in 2023 and has never reported its results. A 2023 review of non-invasive fat removal treats the combination of modalities as an open research question rather than settled practice. So: a reasoned protocol built on a plausible but actively debated mechanism, not a demonstrated synergy. Anyone who tells you otherwise is selling rather than explaining.

Stage three: cryolipolysis, where the fat actually goes away

Cryolipolyse works on a different principle entirely, and it is the only stage that permanently reduces the number of fat cells in the treated area. Fat crystallises at a higher temperature than surrounding tissue, so controlled cooling can injure adipocytes while leaving skin, nerves and vessels intact. The damaged cells then die and are cleared gradually by the immune system.

That clearance is why nothing dramatic is visible on the day. Reviews of clinical efficacy and safety describe the result developing over roughly two to six months, with something in the region of a fifth to a quarter of the fat layer in a treated area reduced. Treat that range as a ballpark drawn from small and largely uncontrolled studies rather than a promise; a recent systematic review and meta-analysis is candid about the quality of the underlying evidence. Reassuringly, repeated same-day treatments have been shown not to disturb serum lipids or liver values.

One complication deserves stating plainly rather than burying. Paradoxical adipose hyperplasia is a rare response in which the treated area enlarges instead of shrinking, months later, and it does not resolve on its own; correction is surgical. The largest multicentre evaluation puts it at roughly 0.43% of patients, while an NIH-hosted clinical reference notes estimates as high as around 2%. It is uncommon, its cause is unknown, and you are entitled to hear about it before consenting rather than afterwards.

Stage four: mechanical drainage, and one claim we will not make

The LPG room: the closing drainage stage, worked through the endermologie suit.

The session finishes with LPG mechanical massage, again over the whole body. The physiology here is genuine and rather elegant: lymphatic vessels are mechanosensitive, and stretching the vessel wall triggers the contractions that pump lymph along. Mechanical stimulation is not merely pleasant; it engages the pump. In aesthetic practice, lymphatic massage is used sensibly for post-procedure swelling and comfort, and vacuum-roller massage has a documented role in the appearance of cellulite and connective tissue.

What we will not tell you is that the massage flushes away the fat released earlier in the session. Clearance after non-invasive fat reduction is a slow local process measured in weeks, and direct blood measurement finds no clinically meaningful lipid load waiting to be swept out on the same afternoon. Drainage earns its place at the end of four hours of treatment for comfort, swelling and circulation. That is enough; it does not need a mechanism it has not got.

What four hours can and cannot do

Every credible authority on this subject says the same thing, and it is worth hearing before booking anything. The American Academy of Dermatology is explicit that non-invasive fat reduction suits people already near their goal weight, targeting a stubborn area rather than replacing weight loss. The American Society of Plastic Surgeons makes the same point about candidacy. And Rox Anderson, the Harvard dermatologist whose laboratory invented cryolipolysis, put the limit bluntly in Harvard Health Publishing: it removes fat you can pinch, and it will not improve your overall health. None of these technologies touch visceral fat, the fat that actually matters metabolically.

It also means the tape measure needs reading with care. Measurements are taken before and after the session, and a same-day change genuinely reflects something — but on the day, part of it is fluid movement and reduced congestion rather than lost fat. The fat-related change is the slow one, arriving over the following weeks as the cryolipolysis stage completes.

In practice the package runs about four hours and focuses on one body area, which is what allows each stage to be done properly rather than gestured at. It costs IDR 2,599,000, includes two cryolipolysis areas, and the studio asks for a day’s notice so the room and protocol can be prepared. A face-focused add-on is available in the same session. If you are near a stable weight and want a specific area refined, that is the situation these four hours were designed for. If the goal is weight loss, the honest answer is that no machine in the room will do it.

Treatments in this article

Forfait Express Slimming — the four-hour session described here, with live booking

Cavitation — the stage with the strongest published mechanism, also available on its own

Cryolipolyse — the only stage that permanently reduces fat-cell numbers

Book it

Combo20 – Quick Weight Loss & Body Contouring Package IDR 4,000,000 — the longer course, if one session confirms the approach suits you

LPG Endermologie Corps IDR 799 000 — the drainage stage as a standalone session

Cavitation 80 kHz: a scientific overview — the ultrasound stage in more depth

Cryolipolysis: non-invasive fat reduction explained — how the freezing stage works and how long it takes

Cold lipo laser: what the evidence supports — more on the opening stage of the protocol

Express Slimming in Bali: four hours, four technologies.
